Good afternoon. I’m _______, and today I’m going to speak to you about social media and breach of confidentiality. As far as the evidence goes, I am going to maintain a professional perspective on privacy and containment of patients.

Slide 2:

For students of nurse education, advanced technology will always present new challenges concerning the law and ethics. Many will use social media and other forms of electronic communication. These methods do present the quickest, most convenient forms of communication; however, they also pose enormous risks regarding the security of information, privacy, and trust. Concerning nurse education, the use of web platforms brings about numerous variables involving HIPAA compliance, professional relationships, and ethics (Ibrahim et al., 2024). Thus, nursing schools must implement digital professionalism and ethics user policy with the flexibility to adapt to new inventions and innovative approaches within the legal and ethical bounds of healthcare education.

Problem Description: Patient Confidentiality Breach via Social Media

Data was posted by a nursing student from a clinical placement at Cadia Healthcare, USA, and shared via social media. It included enough details regarding a specific Cadia Healthcare patient that made it possible for others to identify this patient (Marselas, 2025). Although this student may not have included the patient’s name, this case is a breach of HIPAA, as it goes against the ethical standards of maintaining patient privacy and confidentiality. Not being able to differentiate the professional implications of Internet use, along with the institution’s policies, may result in legal implications, a breach of trust with the client, and a tarnished reputation for the nursing profession. Thus, there is a need for a change of orientation of the employees and nursing students in relation to the use of social media.

Change Theory and Regulatory Framework

Lewin’s Change Theory assists in the gradual alteration of behaviors concerning the management of breaches of patient confidentiality. There are three stages to Lewin’s change theory: unfreezing, changing, and refreezing. The first stage, unfreezing, is to create an understanding of the negative impacts of violating patient confidentiality. In the stage of changing, one is subjected to an educational advocacy program on how to carry out the given policies more effectively. The stage of refreezing is when the new behavioral strategies are sustained through ongoing commitment and support (Majka, 2024). The HIPAA Privacy Rule is an active legislative measure in the U.S. healthcare system to create and maintain patient confidentiality and to ensure that healthcare workers are in compliance with the system (Centers for Disease Control and Prevention, 2024). Furthermore, the American Nurses Association (ANA) advocates for nursing professionals to uphold patient confidentiality and to maintain a high level of professionalism in their practice (American Nurses Association, 2025). Considering everything, these theories and models can be the basis for the development of a behavioral management program.

Comprehensive Change Plan for Confidentiality Breach

This change strategy will unfold in three phases over six months. Phase I will involve assessing the current level of understanding of HIPAA and social media and identifying knowledge gaps through the use of various baseline tools. Phase II will involve the creation of learning opportunities via workshops and a case study policy on digital professionalism (Albabtain et al., 2024). In Phase III, the policy of continuous evaluation will be implemented, and confidentiality will be enforced through faculty champions.

Diversity, Equity, and Inclusion Considerations in Confidentiality Practice

Thinking about social media from an intercultural perspective and considering different ways that people use social media indicates that there are different perceptions and different types of information that social media may include. For example, there are some parts of the world in which social media is almost fully open, and people do not see the need to keep any information, not even personal group information about themselves, private. This brings forth challenges to the expectation of maintaining patient confidentiality, which is a legal requirement. This requirement can create a legal conundrum because these individuals may not be able to fully appreciate the necessity of legal contracts, for example, HIPAA (Tariq & Hackert, 2023).

Not all students, especially those from various backgrounds, may have developed the same technology skills. Therefore, they are less likely to understand some online behaviors that others may consider inappropriate. In addition, students from underrepresented social and ethnic backgrounds tend to have issues accessing adequate information. Thus, they do not gain the necessary understanding of the expected behaviors of individuals in an organizational setting. Consequently, they unknowingly breach such regulations (Vandecasteele et al., 2024). Such issues may be compounded by the absence of instructional materials that portray a variety of social and ethnic minorities and by implicit bias. Most people tend to violate regulations due to a lack of sufficient knowledge and education. To overcome such issues, a different and more specific educational approach has to be adopted.

An example of a culturally specific approach to education may consist of the availability of learning resources in a number of languages, the development of a variety of case studies, and the formulation of tailor-made training programs. Mentoring may enable learners to understand appropriate behaviors in the use of technology and the need to practice information confidentiality. A cultural assessment may include the use of tools that measure the cultural appropriateness of behavior modification (Ulbricht et al., 2024). Hence, the use of techniques that are related to confidentiality in the training of learners would be appropriate.

Evaluation and Sustainability Monitoring Plan

To evaluate the effectiveness of the proposed intervention, an evaluative methodology incorporating both quantitative and qualitative approaches will be utilized. A few examples would include the implementation of knowledge evaluations before and after the intervention to assess if the participants gained knowledge, and the assessment of breach of confidentiality violation incident reports to determine if the number of breaches diminished. The participants’ experiences and opinions related to digital professionalism will be obtained through surveys and focus groups. The proposed intervention will be sustained through the implementation of annual training, policy enforcement, and compliance (Conduah et al., 2025). Evaluation for the proposed intervention will be assessed by a reduction in HIPAA violation breaches, an increase in knowledge evaluation scores, and compliance with institutional policies.

Legal and Ethical Issues Requiring Behavioral Change

When confidentiality is breached on social media, the posting organization may face legal challenges concerning HIPAA violations. Costs related to fines and penalties and the loss of business are the least of the problems that the Office for Civil Rights may impose on an organization for noncompliance with federal rules and regulations. Organizations may be subject to civil lawsuits and reputational damage if they take no proactive measures to avoid the posting of confidential information. One example of this is when a nurse student posted a story on social media that violated HIPAA by disclosing information about a patient.

From an ethical point of view, confidentiality breaches violate the CNA values of individual autonomy and professionalism. The ANA Code of Ethics addresses the violation of confidentiality by stating, “Nurses have a responsibility to maintain the confidentiality of patient … information” (American Nurses Association, 2025). Professionalism and the value of individual autonomy must be preserved in all situations to allow the profession to maintain its image and provide a high level of professionalism. An example of a violation of individual autonomy and professionalism is posting a private case on the Internet. The issue requires the participation of all individuals and organizations involved.

Necessary Changes and Change Management Integration

Behavioral modifications will demand an end to divulging patient information on social media, compliance with the confidentiality policy, and the ability to think critically and ethically. These behavioral modifications will be in line with Lewin’s Change Theory with respect to the use of ‘awareness’, ‘education’, and ‘continuous monitoring’ (Majka, 2024). Furthermore, with regard to the assurance of the sustainability of these behavioral modifications, support from the organization will be necessary.

Effective implementation will demand the combination of educational, technological, and organizational resources. The scientific education in this context refers to learning through the use of simulations, technology in the training of professionalism, and illustrative cases (Ibrahim et al., 2024). Organizational resources allow for the provision and evaluation of this training through techniques such as policy, legislation, Learning Management System, and anonymous reporting (Conduah et al., 2025). The application of the aforementioned resources in this context will address the issues of ignorance and modify behavior.

There are knowledge gaps pertaining to the benefits offered by the digital professionalism training and the relationship between social diversity and confidentiality. Different organizational policies affect consistency, and this is the primary reason for challenges that most institutions face. Some of the challenges that most institutions face can be solved through innovative design.

Values of communication and privacy reveal the level of confidentiality that is appropriate. Based on the documentation gathered concerning laws of confidentiality, there are positive changes in the behaviors of individuals as a result of the social sensitivity training programs (Vandecasteele et al. 2024). Examples from all cultures and social sensitivity workshops will be prepared and held. This will help all individuals learn the appropriate application of the principle of confidentiality.

An all-encompassing methodology to the problem, which encompasses the evidence-based methodology and observes the legal, ethical, and personal/social requirements of the individuals concerned, will allow for the elimination of all possible breaches of confidentiality in nursing education. Such a methodology for addressing the problem can be aligned with Lewin’s Change Model, which permits the building of a favorable environment for the progressive workplace of the present era and the safeguarding of the confidentiality of the patients while offering the required means for addressing the concerns and instructing the prospective leaders.
